Employee database design with Normalization

I need to design a database which should have tables with the below fields,with respect to each normalization rule.

Employee ID,
First Name,
Middle Name,
Last Name,
Date of Birth,
Address Type,
Address Line 1,
Address Line 2,
Address Line 3,
City,
State,
Pin Code,
Type of Identification,
Identification Number,
Issued Authority,
Date of Issue,
Date of Expiry,
Place of Issue,

And An Employee can have any number of address and Identification information.Please help me.
